Practice Nurse (RN or RPN) – Family Practice Clinic

Location: Springwater, ON

Position Type: Full-Time / Part-Time

Start Date: Week of April 15, 2026

About the Role

Our fast-paced Family Practice Clinic is seeking a dedicated, patient‑centered Practice Nurse (Registered Nurse or Registered Practical Nurse) to join our collaborative healthcare team. This role is ideal for a nurse who thrives in primary care, values health promotion, and is passionate about supporting patients across all stages of life.

You will play a key role in delivering high‑quality clinical care, providing patient education, and ensuring smooth, efficient nursing services in alignment with the College of Nurses of Ontario (CNO) standards.

Key Responsibilities

1. Immunization & Clinical Procedures

  • Administer routine childhood vaccinations according to the Public Health Ontario schedule.
  • Maintain accurate electronic immunization records and provide parent education.
  • Perform cervical screening (Pap tests) and counsel patients on cervical health and HPV protocols.
  • Provide wound care, including assessments, dressing changes, and suture/staple removal.
  • Administer IM and SC injections (e.g., B12, allergy shots, Depo‑Provera).

2. Patient Assessment & Triage

  • Conduct initial assessments, including vital signs and health history.
  • Perform phone triage to determine urgency and appropriate care pathways.
  • Assist physicians with minor procedures and physical examinations.

3. Health Promotion & Chronic Disease Management

  • Conduct Well‑Baby and Well‑Child assessments.
  • Support patients with chronic conditions such as Diabetes (including foot care and glucose monitoring) and Hypertension.
  • Provide lifestyle counselling related to smoking cessation, nutrition, and mental health resources.

4.Administrative & Laboratory Duties

  • Maintain vaccine “cold chain” protocols for safe storage and handling.
  • Coordinate specialist referrals and follow up on laboratory results.
  • Perform point‑of‑care testing (e.g., urinalysis).

Qualifications & Requirements

  • Registration: Current General Class registration in good standing with the College of Nurses of Ontario (CNO) as an RN or RPN.
  • Education: Nursing Diploma or Degree; additional primary care/community health certifications considered an asset.
  • Experience: 1–2 years of experience in a primary care or family practice setting preferred.
  • Skills:
  • Proficiency in administering pediatric immunizations.
  • Certification or experience performing cervical screening (Pap tests).
  • Familiarity with EMR systems; TELUS CHR experience preferred.
  • Strong understanding of Ontario’s immunization schedule and Public Health reporting requirements.
  • Certification: Current Basic Life Support (BLS) certification.
