The Role

We are looking for a dedicated and organised Care Co-ordinator to join our team at Your Quality Care Services Limited.

In this role, you will be central to ensuring the smooth and efficient delivery of care services. You’ll manage staff rotas, arrange client visits, carry out assessments and reviews, and help maintain high standards of care across the service. Your work will support both the operational and quality assurance aspects of care provision.

Key Responsibilities:

  • Create and maintain accurate client records and care plans
  • Allocate care staff to meet client needs effectively
  • Ensure all visits are covered during staff absences
  • Carry out client assessments and risk assessments promptly
  • Review and update care packages regularly
  • Monitor staff workloads and raise any recruitment needs
  • Ensure compliance with company policies and CQC standards
  • Supervise care staff and carry out spot checks
  • Attend staff meetings and participate in the on-call rota

The Person

We’re looking for someone who has:

  • NVQ Level 3 in Health and Social Care
  • Experience in care coordination within a domiciliary care setting
  • Strong understanding of CQC requirements and care standards
  • Excellent communication and organisational skills
  • Confidence in writing care plans and carrying out assessments
  • A proactive and solution-focused approach
  • A full UK driving licence and access to own transport (desirable)
  • Experience managing care package costs and handling feedback (desirable)
